Your rating of Vcc up to 25v or 40v is wrong. When running at Vcc = 15v, I experienced a short circuit across the supply in first case and in second case, a bang and a flash of light when a component blew up ! These devices have a 10 volt capacitor across the supply voltage - any attempt to run above 10v will blow this component. This happened twice before I realised the source of the problem. I have replaced the surface mount 10v 47uf capacitor with a 25v 47uf radial capacitor. Once modified
